Follow the instructions indicated on the paper. If it requires capital letters, then be it. If not, then use small caps.

It really depends on what IELTS test result you will send to the university. But if you take the exam again, if I'm not mistaken, the latest result will be your current score and the previous one will not be honored.

Yes, I agree. Both British Council and IDP have the same grading system/criteria when it comes to the IELTS. Both of them also give the same exam. The differences would be on the test fee and venue.
